What it is
Prediabetes — glucose above normal but below the T2D threshold: HbA1c 5.7-6.4% (ADA) or 6.0-6.4% (WHO), fasting 5.6-6.9 mmol/L, OGTT 7.8-11.1 mmol/L. Without intervention, 5-15% per year progress to T2D. Lifestyle (DPP trial) reduces risk by 58%.
Principles
Risk-factor screening
Age > 45, BMI > 25 (>23 in Asian populations), sedentary lifestyle, family history of T2D, prior GDM.
Weight loss
5-7% of starting weight — effect on glycaemia confirmed by DPP. Weight maintenance matters more than speed of loss.
150 min of activity a week
Aerobic + resistance. At least 3 sessions a week.
CGM as an education tool
A single 14-day course shows the real glucose response to one's usual food. Often changes behaviour long-term without a prescription.
