By completing and submitting a contact form (an enquiry, a request for consultation, or a request for a commercial offer) on the website medilife-farma.com, I, in accordance with the Law of the Republic of Uzbekistan «On Personal Data» dated 02.07.2019 No. ZRU-547, freely, of my own will and in my own interest, give my consent to the processing of my personal data by the operator — Medilife-Farma LLC — on the terms set out below.

   • name (the full name or name that I provide);

   • audience category (patient / doctor / clinic / distributor);
   • message text (free-text message that I enter myself).

   I understand that I should not include special categories of data (in particular, health-related information) in the free-text message, and I am responsible for the information I voluntarily disclose in the message.

3. 3. Purposes of processing

   I give my consent to process the specified data for the following purposes:

   • reviewing and handling my enquiry, application or request;
   • providing consultations on the operator's products and services;
   • preparing and sending me commercial offers (CO);

   • file storage — Cloudflare R2;
   • communication channels — Telegram, phone and email;
   • aggregated website analytics that do not identify a specific individual.

   As some of these services are located outside the Republic of Uzbekistan, I consent to the cross-border transfer of my personal data to the extent and for the purposes provided for in this consent. The consent also covers the use of strictly necessary (technical) cookies and aggregated analytics.

   Data is not transferred to third parties for purposes other than those stated without separate consent, except in cases expressly provided for by the legislation of the Republic of Uzbekistan.

6. 6. Validity period of the consent and the withdrawal procedure

   This consent is valid from the moment the form is submitted until the processing purposes are achieved or until I withdraw it.

   Personal data is stored until the processing purpose is achieved or until consent is withdrawn, unless a different (longer) storage period is established by the legislation of the Republic of Uzbekistan.

   I have the right to withdraw this consent at any time by sending a written request to info@medilife-farma.com or to the operator's address specified in section 1.

   Upon receipt of the withdrawal, the operator ceases processing and deletes (destroys) the data, except for information that the operator is required to retain by law.
